Release Forms
At A&O, we do everything we can to ensure that each student is protected and safe while they are under the care of First Pres A&O staff. Sometimes we require parental consent/release forms for our activities. All students are also responsible for reflecting the attitudes and character of Christ while participating with A&O. You may either click on the form you need below and print it out, or pick one up from Alana in the A&O offices. Parental signatures are required.
